Because these mortality reductions preceded effective medical therapies, McKeown argues that they must have been driven by environmental changes, particularly improved nutrition brought about partly by economic growth and the associated increases in food supplies, and to a lesser extent by improved public-health infrastructures such as public water and sewer lines.

At its early stages, typhoid’s symptoms often resemble those of respiratory diseases, and pneumonia is often present. In nearly all cases, typhoid victims experience severe fever. Body temperatures can reach as high as 105 F. A week or so after incubation, rose-colored spots sometimes appear on the patient’s abdomen. 8 Three weeks after incubation, the disease is at its worst. The patient is delirious, emaciated, and often has blood-tinged stools. One in five typhoid victims experiences a gastrointestinal hemorrhage.
